Twilio can support HIPAA workloads, but a signed BAA is only one part of it.

What Twilio requires

  • The right edition. Twilio states that HIPAA accounts require its Security Edition or Enterprise Edition.
  • A signed Business Associate Addendum, arranged through Twilio sales or your account manager.
  • Eligible products only. Twilio publishes a list of HIPAA-eligible products. Check that the messaging products you plan to use are on it.
  • Your architecture. Twilio calls HIPAA a shared responsibility. The BAA does not make your setup compliant by itself.

Designing messages

  • Keep medication and condition names out of texts. Lock screens and shared phones are public.
  • Use links to a logged-in portal for anything clinical.
  • Route every reply that mentions symptoms or side effects to the clinical partner, immediately.

Consent and registration

Marketing texts need their own documented consent, separate from care and transactional messages, and US business texting generally requires 10DLC registration or a verified toll-free number. Budget a few weeks for registration before launch.
